Community Support Program (CSP)

  • Community Support Program (CSP) consists of mental health and substance use rehabilitation services and supports necessary to assist an individual in achieving and maintaining the highest degree of independent functioning. The service utilizes a team approach to provide intensive, rehabilitative community support, crisis intervention, individual, and group psycho-education and skill building for activities of daily living, peer support and self-management.

    CSP includes a comprehensive array of rehabilitation services most of which are provided in non-office settings by a mobile staff. Services are focused on skill building with a goal of maximizing self-management skills and independence. Community-based treatment enables the team to become intimately familiar with the individual’s surroundings, strengths and challenges, and to assist the individual in learning skills applicable to his/her living environment. The team services and interventions are highly individualized and tailored to the needs and preferences of the individual.

    CSP services focus on building and maintaining a therapeutic relationship with the individual while delivering rehabilitative, skill building interventions and activities, and facilitating connections to the individual's community recovery supports.

  • Pathways is currently accepting new clients to the CSP Program.

    Fill out the application and submit it to Pathways.

    A representative will contact you once an application has been submitted.

Mental Health Waiver (MHW)

  • The mental health waiver program operated by the Advanced Behavioral Health, DMHAS, and the Department of Social Services (DSS) for Medicaid individuals with serious mental illness who either live in a nursing home or are at risk of institutional placement.

    The mental health waiver program permits Connecticut to provide a comprehensive array of home and community-based services that enable individuals to live safely in the community. Individualized recovery plans developed with each person include rehabilitative or support services, Community Support Program and Recovery Assistant.

    • You must have a documented severe mental illness

    • You must live in Stamford or Greenwich

    • Must be 22+ (18 to 22 has young adult services through DMHAS)

Support Groups

Family & Friends Support Group

Every third Wednesday of the month

7:00-8:00PM

This group is for friends and family members who have a loved one with a mental illness. Facilitated by one of our case managers, this group aims to use our similar circumstances to gain perspective, insight and strength.

Meant for people living with mental health issues, this group is a confidential and judgement free zone. While many of the hardships we face can feel isolating, there are others with similar experiences. This group is a way to connect, build community and learn from one another.
